Statement: The Trustees, Directors, and Staff at Durham City Shopmobility are committed to providing the best possible service to our members. The Health and Safety of our members is one of our top priorities. With this in mind, we would like our members to be aware of the following:
Anyone with a medical condition such as epilepsy, diabetes, and who is subject to seizures or angina attacks, or whose sight is impaired to the extent that they should not drive, should use our attendant-controlled wheelchairs for their own and public safety.
If you have been previously advised not to use an electric wheelchair and/or scooter by your GP, physiotherapist, or occupational therapist—or have been refused such a piece of equipment during an assessment—we strongly advise against using our equipment.
Our standard wheelchairs and scooters have a maximum weight limit of eighteen stone. Specialised equipment is available upon request.
All wheelchairs are fitted with lap belts. Differing professional opinions exist regarding the use of such belts. Our staff will explain these issues to you, and the use of belts will then be at the discretion of individual members.
I acknowledge that I have undergone a basic introduction to the safe use of the equipment and have had the controls explained to me. I have also had a supervised trial run on the equipment.
I accept full responsibility for any wheelchair or scooter in my care while using Durham City Shopmobility services. Specifically, I agree to the following:
